Check old check registers/online banking statements to see if payments were made to any insurance companies during this and the preceding year. Ask past/present employer if there were an employer-provided policy and, if so, which carrier issued it. Ask your insurance agent and attorney, if your family has them. Check income tax returns which might identify interest income. Finally, the Medical Information Bureau (MIB) has a lost policy locator service that, for a relatively reasonable fee, might be able to assist.
